Pakistan brought in recalled paceman Umar Gul for Sohail Tanvir after the right-arm paceman was flown in as cover for Junaid Khan, who was also fit to play.
Gul, 29, plays his first match since undergoing a knee operation in May this year.
Sri Lanka remained unchanged from the side which won the second match in Dubai by two wickets on Friday.
The five-match series is tied at 1-1 after Pakistan won the first match by 11 runs in Sharjah.
